Vaccination records must be submitted prior to the start of the first class.
They may be faxed directly from your veterinarian, emailed, mailed separately or hand delivered.

Any dog which, in the opinion of Dog Obedience Training LLC, is uncontrollable, unpredictable, aggressive,
or in any way a danger to people or other dogs will have to be muzzled at all times while on the premises or
be expelled. Muzzles are not allowed in Rally Obedience classes.

Refunds will be given up to 48 hours prior to the start of class; No refunds will be given after that
time. Missed classes cannot be made up. However, classes cancelled by Dog Obedience Training LLC
because of weather or any other uncontrollable circumstance will be compensated for by free admission to
one of our open floor times.

All applicants will be responsible for cleaning up after their dog. The grassy area across from our building
does not belong to us; PLEASE KEEP IT CLEAN. Indoor potty facilities are available in the training room for
your convenience. Your cooperation will be greatly appreciated.

By submitting this application, I hereby acknowledge that dog-training activities are not without risk. I further
acknowledge that I personally, am solely responsible for my dog’s behavior and actions at all times. I
expressly assume the risk of any damage or injury to myself, my dog or to any other person accompanying
me while on or off the premises. I absolve Dog Obedience Training LLC, Gone to the Dogs LLC and Gone
to the Dogs Daycare LLC along with all persons associated with the above, from any responsibility for any
harm to persons, property or animal while on or off the premises for any reason. I also hereby indemnify and
hold harmless release Dog Obedience Training LLC, Gone to the Dogs LLC and Gone to the Dogs Daycare
LLC along with officers, employees, members and agents from any and all claims, or claims by any member
of my family or claims by any other person accompanying me.

We must have current records on file at all times.
